Statistical misinterpretation arising from failure to consider the base rate. Enormously common, especially in situations where the base rate is very low.
For example, if a large majority of the population is vaccinated against the flu, then even if the vaccine is very effective, most people who arrive at hospital with the flu will be vaccinated.
